    Provide all information to the best of your ability. Payments by cash or check can be mailed to the Fort Atkinson Area Chamber of Commerce at 244 N. Main Street, Fort Atkinson, WI 53538. Checks should be made out to the chamber. Credit card payment may be accepted over the phone Monday-Friday 8am-5pm at (920)563-3210. Please contact us if you have any questions. Printed applications may also be picked up at that chamber's office. PLEASE RETURN BY March 31, 2021. After that, late fees apply.

    Prepared Food Products: Vendor must have photocopies of all necessary licenses for the production of the processed product and must have them filed with the Market before offering any processed item for sale. Some foods are license exempt. Licensing questions should be addressed to the State of Wisconsin Department of Agriculture, Trade and Consumer Protection. Each vendor is responsible for making sure their products are compliant with state and national laws.

    Please help us select the best date for our annual pre-season vendors meeting by selecting the date that works best for you. We will hold the meeting on the date that the most vendors can attend and will post a recording of the video online for those to watch later who might not be able to attend the event. An email will be sent to vendors to notify them when the date is selected. Please contact the market manager if you have any questions.

    The Fort Farmers Market strives for a well-balanced offering of vendors committed to attending the market EACH SATURDAY during our outdoor season. Additional DAILY vendors are welcome as space allows on a first-come basis. Extra vendors will be accepted on Arts Saturdays to accommodate more artists/artisans.
    SELECTION OF MARKET DATES
    Please check all dates below that you intend to sell during our 2021 season. If you select "I do not know yet.", you are agreeing to notify the market manager by the Wednesday before you intend to sell. Daily producer spots are assigned on a first-come-first-serve basis in relation to our market's product needs. Seasonal producers who miss more than 3 consecutive Saturdays without 24 hour notice will be considered "daily vendors" unless otherwise arranged with the market manager and may lose their booth placement.

    This year, Blodgett Garden Center has expressed an interest in partnering with Fort Farmers Market to host two evening-markets in their outdoor garden center and greenhouse. These markets will focus on Arts and Crafts vendors, but will also include food and produce vendors as space allows. The participation fee is included as a benefit for seasonal vendors or $20 per event for daily vendors. Please indicate your interest in participating in these events.

I further acknowledge full responsibility for all my activities (and those assisting me) in the Farmer’s Market throughout the term of this permit. I understand that violations of these Policies & Procedures may result in my being barred from further consideration. I also understand that the Fort Atkinson Area Chamber of Commerce and the City of Fort Atkinson do not carry any insurance policies to cover individual participants and that I am hereby advised to consider carrying my own general liability and product liability policies. The FAACC reserves the right to modify the policies as needed at any time. Seller agrees to indemnify and hold the FAACC and the City of Fort Atkinson harmless from any and all claims and liabilities. *
