    Guardianship Representative PT

    The Guardianship Representative serves as an advocate to all wards appointed by the State of Wisconsin Circuit Court and is responsible to provide professional management of the ward’s residence, provision of education, health and general welfare, consent to medical treatment and other personal, legal and medical affairs.
    Advocating on behalf of individual to obtain necessary care, visit and contact with wards to ensure appropriate services are being provided, apply application and follow-up process for services, and perform assessments to determine eligibility for services are key factors in the provision of this exceptional service.
    Essential Duties & Responsibilities include the following but are not limited to:
    Advocate on behalf of the individual to obtain the necessary care, services and/or placement to ensure the individual's safety and dignity.
    Maintain working knowledge of and adhere to Wisconsin State Statutes governing guardianship services including Ch. 51, 54, 55 and Administrative Code 85.
    Guardian of Person
    Manage a case load of wards under guardianship with responsibilities including but not limited to:
    monitoring the ward is living in the most appropriate environment that addresses the ward's goals, needs, and preferences.
    collaborate with the wards’ team in a solution driven manner for the best interest of the ward
    Guardian of Estate (real and personal property, tangible and intangible, and includes anything that may be the subject of ownership)
    •Manage a case load of wards under guardianship with responsibilities including but not limited to:
    accurately maintain and monitor monthly billing of ward fees for accountability of completion
    review wards bills for accuracy and submit to the business office for payment in a timely manner
    Agency or as assigned:
    Demonstrate ability to work independently with minimum amount of oversight and direction
    Represent Opportunities, Inc. in a professional and courteous manner to wards, families, team members, externals stakeholders, legal and medical partners, community agencies, etc.
    Possess acute awareness of community resources and funding options.
    Assist with marketing and expansion of the program and to work closely with referral sources to ensure their satisfaction with the program.
    Job Type: Part-time
    Salary: $21.00 - $25.00 per hour
    Expected hours: 20 – 25 per week
